MEDIA RELEASE AGREEMENT

I hereby affirm my student’s voluntary participation in Grace Weber’s Music Lab. I understand that he/she/they may appear in photography and video content that will be produced and recorded by 88Nine Radio Milwaukee WYMS-FM and/or Grace Weber’s Music Lab for advertising or promotional purposes. I authorize Grace Weber’s Music Lab and/or 88Nine Radio Milwaukee to record on film, videotape or otherwise my student’s name, likeness, image, voice and performance, and any materials (such as videos, photographs, soundtracks, etc.) my student makes available to Grace Weber’s Music Lab and/or 88Nine Radio Milwaukee. I grant Grace Weber’s Music Lab and/or 88Nine Radio Milwaukee a non-exclusive, perpetual, irrevocable, royalty-free, transferable, sub-licensable, worldwide license to exercise now or at a later date to the content, in full, in part, and in all possible media (now known or later developed). The foregoing rights include, but are not limited to, the right to display, perform, broadcast, and publish in public or private sites, newspapers or other media and reports and so on, all or part of the content and other information your student provides.

I hereby release Grace Weber’s Music Lab and/or 88Nine Radio Milwaukee showing or distributing the content from any claim by me or my student for damage to our person, property, or reputation, or for invasion of privacy. I affirm that to the best of my knowledge all material authorized, furnished and/or used by my student in these recordings is my student’s own original material or material which my student has full authority to use for such purposes. I further affirm that Grace Weber’s Music Lab and/or 88Nine /Radio Milwaukee are the owner of all rights in and to said content and that no monetary consideration is due and owing myself or my student.
